In February 2024, I published Future Healthcare 2050: How AI Transforms the Patient-Physician Journey to provide a roadmap for healthcare professionals, executives, and innovators facing the rapid emergence of artificial intelligence. Four months later, the World Economic Forum released its white paper, “Earning Trust for AI in Health,” calling for responsible AI development that centers on transparency, inclusion, and trust. While I welcome the WEF’s contribution to the conversation, its timing and omissions highlight a deeper issue in how we discuss AI in healthcare: the absence of the clinical voice, a voice that is crucial and should be at the forefront of AI policy discussions.
The WEF paper is thoughtful, well-researched, and directionally aligned with many of the themes I wrote about. But the conversation it leads—like so many others in global health policy—remains dominated by institutions, technology vendors, and academic elites. What remains missing is the voice of the physician, the nurse, the clinician at the point of care. Even more absent is the voice of the patient, whose trust we all seek to preserve. The potential consequences of AI policies that do not consider these voices are significant, including the erosion of patient trust, the dehumanization of care, and the potential for harm.
Policy Without the Bedside
The WEF report outlines five principles for trustworthy AI in health: human agency, accountability, explainability, fairness, and system-level transparency. These are essential values. But values are not policies. And policies that are not grounded in real-world clinical workflows are destined to fail—or worse, cause harm.
In Chapter 13 of Future Healthcare 2050, I described how digital tools often weaken the patient-physician relationship. When AI systems insert themselves without regard for the human dynamics of care, they risk reducing patients to data points and clinicians to passive users. No principle of “explainability” can overcome an implementation that interrupts eye contact or adds cognitive burden to an already exhausted nurse.
Clinical reality is messy. Patients do not follow clean datasets. Clinicians are not bureaucrats executing machine judgment. Until this truth informs policy frameworks, they will remain abstractions—well-meaning, but detached.
The Ethical Gap
The WEF rightly calls for fairness and inclusivity, but stops short of grappling with clinical ethics. In Chapter 6 of my book, I outlined how AI can amplify bias, obscure accountability, and erode shared decision-making. Yet, most governance models address this from a data science or legal perspective, rather than from the bedside moral lens that guides medical care.
Trust cannot be mandated solely through oversight. It must be earned—through behavior, transparency, and outcomes. Our regulatory frameworks must embrace ethical vigilance, not just compliance checklists. This vigilance will ensure that AI in healthcare is developed and deployed with the highest ethical standards, providing reassurance and confidence in its potential.
From Principles to Practice
Chapter 16 of Future Healthcare 2050 presents a governance model grounded in healthcare leadership, rather than vendor power. It places physicians, nurses, and patients at the center of AI implementation, ensuring that their voices are heard and their needs are met. It builds in feedback loops, interdisciplinary ethics reviews, and post-deployment outcome tracking to continuously evaluate and improve the impact of AI on patient care. These are not radical ideas. But they are rarely found in global white papers because they require relinquishing some degree of central control in favor of localized, contextual intelligence. This model can address the issues with current AI policies by ensuring that they are not only technically sound but also ethically and clinically responsible.
The WEF’s strength is in setting the agenda. However, the real work—designing and deploying AI that fosters trust—must occur at the grassroots level. There, clinicians must have tools that respect their expertise, patients must feel empowered rather than analyzed, and systems must be evaluated on human outcomes, not just computational accuracy.
A Call for the Missing Voice
The future of healthcare AI will not be decided in Geneva or Washington alone. It will be shaped in exam rooms, emergency departments, and community clinics. It will depend not only on policy, but on culture—on how we design for trust, and for whom.
That voice—the one of the clinician at the bedside and the patient across from them—is still missing from too many global frameworks. Future Healthcare 2050 was my attempt to elevate that voice.
Now, as others begin to echo these themes, it is time to connect the dots—to ensure the momentum behind responsible AI policy is matched by practical, grounded insights from those delivering care.
